Applying for a UofO Associate ID Card

If you will be serving as a representative of your particular faith tradition for an extended period of time, you can apply through the RDA to receive a University of Oregon Associate ID Card.

Only fill out the info below if your faith tradition is an active organization in good standing with the RDA, and you are a Religious Director assigned to attend the regular meetings of the RDA as support staff to university students (forms filled out by others will be discarded).
